Death certificate or authorization for repatriation of remains Application

Required documents

  • A certificate from the Funeral Home stating that the remains have been embalmed in accordance with the sanitary conditions established by the Canadian Government and that the remains pose no health threat to the environment
  • A copy of the consular card
  • A copy of the hospital's medical record attesting to the person's death and its cause
  • A copy of the original birth certificate of the deceased or a legalized copy
  • A flight reservation or a copy of the flight itinerary confirming the intention to repatriate the remains to the Republic of Congo
  • A handwritten and typed letter signed by the family representative in Canada, addressed to the Chargée d'Affaires a.i, requesting the issuance of a death certificate or an authorization for the repatriation of the remains
  • A legible copy of the Funeral Home's license responsible for the remains
  • An official identity document of the representative of the family of the deceased
  • An official letter from the Funeral Home confirming their responsibility in the administrative procedures for the repatriation of the remains
  • Proof of Congolese citizenship such as Congolese passport, Congolese national identity card or certificate of Congolese citizenship of the deceased
